How To Apply:
Application Form: Interested candidates can obtain the application form as well as prospectus in person from the State Ayurveda College and Hospital, Lucknow, against a payment of Rs 1500/- (Rs 1000/- for SC/ST candidates)
The respective amount for application form should be paid in the form of demand draft/Banker’s cheque drawn in favour of Principal, State Ayurveda College and Hospital, Lucknow
Candidates desirous of getting the application post have to add Rs 50/- extra as postal charges
Application Submission: The duly filled in application form along with requisite enclosure documents should be submitted to Office, State Ayurveda College and Hospital, Lucknow, Tulsidas Marg, Turiya Ganj, Lucknow-226004 latest by 03rd October, 2012, upto 05.00 p.m
